Got this from the net:
1: Start out by opening up your image in photoshop:


2: Click the quickmask mode button (circled in red):


3: Select the gradient feature:


4: Make sure that under gradient, you have chosen the foreground to transparent option:


5: Draw a line in the direction you want your transparency; I drew my line upwards, which mean the top part of my image will be transparent:


6: When you let go, there will be a red gradient over your image:


7: Then, click back to standard mode:


8: Hit delete, and voila, your image fades to transparent:


9: Feel free to add a background layer or whatever you want:


However long your line in step six is will be how much of your image will be faded (when you hit delete in step eight.
