Ok First u open a new file..
After that paste a pic. Example this pic:
http://www.hilarypics.com/displayimage.php?album=44&pos=2
http://www.hilarypics.com/displayimage.php?album=44&pos=0
and then paste... put them together.. but be sure the pic has the same background so it would look like a blend. and there u have it! mine turned out like this:

put the decoration and text and there u have it!
